Free Health Checks

The male and female doctors at Yarra Medical in Richmond can provide you and your family with a comprehensive range of free health checks that are fully bulk billed if you have a valid medicare card. Health checks are important as they are designed for early detection of health conditions. Early detection allows for early treatment or prevention.

Health assessment for people aged 45 to 49 years who are at risk of developing chronic disease

(Two single consults required)

Over two appointments this health assessment will assess patients who are at risk of developing a chronic disease. A health assessment at this stage of life can assist patients to make the necessary lifestyle changes to prevent or delay the onset of chronic disease. Patients are only eligible for one of these assessments. Components of the health assessment can be found here.

Health assessment for people aged 40 to 49 years with a high risk of developing type 2 diabetes

(Two single consults required)

The aim of the type 2 diabetes risk evaluation is to support medical practitioners to address the health needs of patients 40 to 49 years of age who are at high risk of developing type 2 diabetes. The type 2 diabetes risk evaluation is a review of the risk factors underlying the patient’s ‘high risk’ score as identified by the Australian Type 2 Diabetes Risk Assessment Tool (AUSDRISK). It includes initiating interventions to assist with the prevention of type 2 diabetes. The assessment is available every 3 years and eligible patients must be aged 40 to 49 years (inclusive) or 15 to 54 years (inclusive) for Aboriginal and Torres Strait Islander people.

Most patients at high risk of type 2 diabetes, heart disease or stroke will qualify for the Life! program with and AUSDRISK score ≥12, an Absolute Cardiovascular Disease Risk Assessment score ≥10%, or if they have one of the listed pre-existing conditions which are known to place them at increased risk of CVD or type 2 diabetes. This program is free for most patients and aims to modify their lifestyle to prevent the onset of disease and reach their goals for a healthier life. Health assessment for people aged 75 years and older

A health assessment of an older person is an in-depth assessment of a patient aged 75 years and over. It provides a structured way of identifying health issues and conditions that are potentially preventable or amenable to interventions in order to improve health and/or quality of life. Comprehensive medical assessment for residents of residential aged care facilities

A comprehensive medical assessment is a review of the resident, including assessment of a residents health, physical and psychological functioning. The comprehensive medical assessment is not available to residents who are receiving respite care. The assessment is available on admission to a nursing home then annually thereafter. Health assessment for people with an intellectual disability

This health assessment is for patients who have significantly subaverage general intellectual functioning (two standard deviations below the average intelligence quotient (IQ)) and who would benefit from assistance with daily living activities. The health assessment provides a structured clinical framework for medical practitioners to comprehensively assess the physical, psychological and social function of patients with an intellectual disability and to identify any medical intervention and preventive health care required. Health assessment for refugees and other humanitarian entrants

This health assessment is for refugees and other humanitarian entrants who arrive in Australia with complex and unusual medical conditions resulting from their area of origin or previous living conditions. This assessment is separate from, and in addition to, a medical assessment specifically for the grant of a Refugee or Humanitarian visa. The health assessment is a voluntary one-off service and must be provided within twelve months of the person’s arrival in Australia or grant of visa. Medicare Health Assessment for Aboriginal and Torres Strait Islander People

The aim of this MBS health assessment item is to help ensure that Aboriginal and Torres Strait Islander people receive primary health care matched to their needs, by encouraging early detection, diagnosis and intervention for common and treatable conditions that cause morbidity and early mortality. This health assessment is an annual service. The minimum time allowed between services is nine (9) months. Components of the health assessment are listed by age. GP Health Assessment for Former Australian Defence Force (ADF) Personnel

The aim of this health assessment is to help medical practitioners identify and diagnose the early signs of physical and/or mental health problems among former serving ADF members. The health assessment is available to all former ADF members, including former serving members of permanent and reserve forces. It can be performed at any point after the patient’s discharge from the ADF. This health assessment may only be claimed once by an eligible patient. Medication Management Reviews

The Domiciliary Medication Management Review (DMMR) MBS item 900, also known as a Home Medicines Review (HMR), is an MBS item for patients living in the community setting. The goal of a DMMR is to maximise an individual patient’s benefit from their medication regimen, and prevent medication-related problems through a team approach, involving the patient’s GP and preferred community pharmacy. Patients are asked to collect all stored medications at home and bring them into their appointment with their nominated GP. The DMMR referral form can be found here.

Residential Medication Management Review (RMMR) MBS item 903 is also available for patients living in residential care. Reviews are available once only in each 12 month period, however earlier reviews are possible if there has been a significant change in the patient’s condition or medication regimen.
