UBER Driver Medical Assessments
UBER driver medical assessments are available with all doctors at Yarra Medical. Please call reception to book a 20-30 minute consultation for this assessment. Please bring and complete all relevant paperwork before your assessment. If you wear glasses please bring them for your eye test. If you are new to the clinic, please bring a copy of your file from your previous treating doctor. Alternatively, you can visit the clinic to complete a file transfer request from your previous doctor.
Workplace and Insurance Medical Assessments or Reports
Workplace and insurance medical assessments and reports are available with all doctors at Yarra Medical. Please note an extended 20-30 minute consultation is required for these assessments. Please call the clinic to request this appointment. Please bring all relevant printed and completed paperwork for your workplace or insurance provider. If you are new to the clinic you will need to bring a health summary from your previous treating doctor.
VicRoads Medical Assessments
VicRoads medical assessments are available with your regular doctor. If you are new to the clinic you will need to bring a copy of your file from your previous treating doctor. If you wear glasses, you will need to bring them to complete your vision testing.
