Looking for an UBER Driver Medical Assessment in Melbourne?

Uber driver medical assessments are available with all doctors at Yarra Medical. Please call reception to book a 20-30min consult for this assessment. Please bring and complete all relevant paperwork before your assessment. If you wear glasses please bring them for your eye test. If you are new to the clinic, please bring a copy of your file from your previous treating doctor. Alternatively, you can visit the clinic to complete a file transfer request from your previous doctor.

Workplace and Insurance Medical Assessment or Report

Workplace and Insurance Medical Assessments and reports are available with all doctors at Yarra Medical. Please note an extended 20-30min appointment is required for these assessments. Please call the clinic to request this appointment. Please bring all relevant printed and completed paperwork for your workplace or insurance provider. If you are new to the clinic you will need to bring health a care summary from your previous treating doctor.

Vic Roads Medical Assessment

Vic roads medical assessments are available with your regular GP. If you are new to the clinic you will need to bring a copy of your file from your previous treating doctor. If you wear glasses, you will need to bring them to complete your vision testing.

Taxi Services Commission Medical Assessment

Taxi services commission medical assessments are available from all doctors at our clinic. They take 20-30min and we need to know your past medical history, so if you are new to the clinic please bring a copy of your notes or summary of your health from your previous doctor or clinic. If you wear glasses, you will need to bring them to complete your eye testing.
